Sacred Space - Chapel of Intentions

I am asking for prayers regarding my upcoming wellness assessment. A preliminary profile was submitted by one of the physicians, who has never dealt with me, and it is full of astonishing errors and misinformation. I find myself wondering if this person reviewed the wrong chart. I have written a response which I will present to my treating physician, but this situation has me very concerned.
Our Lord told us not to worry. About anything. I am having trouble with that right now, and I ask your prayers for my peace of mind and a good resolution of this fiasco.
